To all PHPA Members:
We wish to announce a positive change that has been made for both the AHL and the ECHL Prescriptions Program. Effective October 1, 2022, we will be moving the plan from our current carrier (Express Scripts) to our new carriers (CVS Caremark for U.S. claims & NexGen for CDN claims).
All claims incurred prior to and including September 30, 2022, with dates of service of October 1, 2021 through to September 30, 2022 that were not hockey related, and providing you were insured at the dates of service, must be submitted to Express Scripts no later than December 31, 2022. Thereafter, we will not be able to honor these claims.
What does this mean for you?
Starting October 1, 2022, our prescription coverage with the new carriers will begin, with CVS Caremark through Meritain Health for all U.S. prescription claims while in the U.S. and NexGen for all Canadian prescription claims, while in Canada. Detailed coverage information will be provided during the PHPA Fall Enrollment meetings to introduce the new plan.
This change will affect all AHL / ECHL prescription claims &/or prescription cards you may have currently, beginning October 1, 2022. We encourage you to advise your pharmacy that the prescription benefits carrier has changed as you will need to provide a new prescription ID card as proof for processing of any future claim(s).
Please note that active players will be provided temporary cards until the PHPA receives your completed Enrollment (Player Authorization) form. Once forms are submitted back to us, they will be entered into the PHPA database and we will thereafter direct the insurance company and prescription carriers to forward permanent medical and prescription cards.
These prescription cards will make it more convenient for both AHL and/or ECHL players who either play in the U.S. or in Canada to use at point of sale, directly through the pharmacy.
Please remember that any changes in status from your last enrollment – whether it be your dependents, address change, beneficiary information, etc., should always be reported to PHPA’s Benefits Dept. not the insurance company. In turn, PHPA will enter the changes and feed that information to them.
